Isaac is 44 years old and was born with profound and multiple learning disabilities after a difficult and prolonged birth resulted in anoxia (absence of oxygen). His parents were told to put their baby in institutional care and “try again” so Isaac grew up in the local learning disability hospital. His parents maintain contact by visiting at Christmas, on birthdays and attend his annual service review. Isaac has cerebral palsy, epilepsy and scoliosis. He needs full assistance with all of his personal, health and social care needs. Fifteen years ago the hospital closed and Isaac was moved out into a small group home with 6 other adults with similar needs, where he is supported by learning disability nurses. Despite significant health issues, Isaac is supported to pursue his interests by staff. Isaac has a wicked sense of humour and loves to watch people “clowning around”. Staff notice that he also enjoys being outdoors, animals and rock music.

Nursing practice

Use the nursing process to help you frame how you will discuss the nursing practice /serice user or client . Use the lecture notes provided from nursing process lecture on 13/11/12 to help you with this. NB do not quote the lecturer but the references they provide in the lecture notes.

So for example after assessing your chosen case study, you would prioritise and plan with rationale the nursing care you would provide. You would go on to implement that care and then evaluate its effectiveness.

Remember to reference everything you say!!

Health surveillance and Health promotion

Bring in epidemiology here which” is the study of diseases in human populations “page 1 (Bonita; Beaglehole and Kjellstrom ( 2006 ) . The epidemiology information on unilearn will be useful.

The World Health Organisation will provide global statistics on most diseases or illnesses and you could also look at the office of national statistics for UK statistics and look at the incidence and prevalence of a disease eg. how many children suffer from asthma in the UK ( incidence ) and are the numbers growing ? (prevalence)

You should look at the concepts of health surveillance and of health promotion. Refer to lecture on 20/11/12 and look at areas within your case study where health surveillance should be carried out and where an opportunity to promote health could take place. How could you monitor the health of Issac or Jonah or Hattie or Albert and then how could you promote their wellbeing and health.

Should also look at inequalities in health here eg. Financial, gender, class etc….. etc…. Lecture onthis subject 27/11/12

Sociological aspects of health and ill health

Lecture on this subject 14/11/12

Sociology – “the study of human social life” page 138 , Naidoo and Wills ( 2008)

What sociological issues have an impact on health or an individual’s perception of health? For example, isolation; class; stigma; perceptions; stereotypes, gender, ethnicity etc …… etc…..

Relate this to your chosen case study and discuss how any identified issues can be addressed. How as nurse can you address these issues? For example how can the stigma and isolation of illness such as depression addressed?

Psychological aspects of health and ill health

Lecture on this subject 21/11/12

What are the psychological affects of illness? For example the anxiety that an illness can produce or a general change in mood; fear of mortality; fear of a loss of independence and self worth etc…. or conversely relief that a diagnosis can bring and offer a person something to work with or fight against.

Again read around this subject and look at the lecture notes on this

Relate this to your particular case study and discuss how as a nurse you might address these issues.

An example of this might be in the case of Albert he has a irreversible chronic respiratory illness that ultimately may shorten his life expectancy. How will he cope with this? How can you help him come to terms with this?

Do not forget to provide references for all your comments and discussion.

Conclusion 200 words (approx)

Draw together all the main points you have previously discussed.

You are wrapping up the case study and bringing it to a logical conclusion

Again references are essential
