PRACTICE SITE PROFILE: Describe the organization overseeing the project(s), its purpose
and/or mission statement, and funding sources. (2-3 pages)
METHODS: What resources were required to complete your practicum project (time, personnel, funds, computers, etc)? What preliminary tasks
were required before you started the project (ex. IRB approval, acquiring an adequate sample size, meetings with community members, etc). What specific tasks were completed to accomplish your project goals? (2-3 pages).
EVALUATION: Assess and provide detail on whether the project activities and
goals were appropriate in addressing the problem. Did they result in the selected the outcomes of your practice project? If they were not successful, explain the reasons why and what you think
should have been done differently. (1-2 pages).
DISCUSSION: Describe your role in the project, including activities in which you were involved. Include discussion of barriers that were identified and how you addressed them This is a personal
discussion of the project from your viewpoint. Thus, it is acceptable to write in the first person (using “I”) for this section.(3-4 pages).
PERSONAL ASSESSMENT: Describe how this Practicum has contributed to an understanding
of public health practice. Identify which of the Public Health Competencies* were applied during the MPH Practicum. these competencies are ” analyze determinants of health and disease using an ecological framework, describe the legal and ethical bases for public health and health services, apply the principles of program planning, development, budgeting, management, and evaluation in organizational and community initiatives, apply basic principles of ethical analysis (e.g. the public health code of ethics, human rights framework, other moral theories) to issues of public health practice and policy. Include at least two of the discipline-specific and four of the crosscutting competencies. How would you grade your performance? Why?
